CONCEPT CARE DISCOUNT PLAN Is our comprehensive eye care plan offered to families and small businesses (including employers, employees and their family members). If you do not currently have a vision care plan, or cannot afford another, please consider this one. You may be referred to an eye specialist of your choice for some medical services - contact your insurance company directly for information to such benefit coverage if it applies to major medical insurance.
What's Covered?
- Our routine eye exam for glasses or same with contact lenses. [One annual per covered person.]
- A discount on eyeglass frame and lenses or frame, lenses, and contact lenses together.
- Savings on medical visits and other eye care related procedures:
- Treatment for pink eye, 'allergy eye', glaucoma, etc.
Includes any follow-up visits.
- Foreign body removal: objects such as metal or wood chips, misdirected eye lashes, etc. [This can often be done in our office.] - Comprehensive eye exams: an eyeglass perscription, glaucoma screening, cataract evaluation and pupillary dilation if warranted.
